The first step to declaring bankruptcy is to complete the required application forms. There are two forms that you will need. The first is called the Bankruptcy Petition (known as form 6.27). The second and larger of the two forms is called the Bankruptcy Statement of Affairs (form 6.28).

The most important form to complete is the Statement of Affairs. This document is 30 pages in length and asks you questions about your debts, income, living expenditures, assets including any vehicles and property. The form also give you the opportunity to explain who your debt problem has come about.
